End of Life Directions for Aged Care (ELDAC) is a project that seeks to improve the care of older Australians through palliative care connections and advance care planning activities. The ELDAC project is funded by the Australian Government Department of Health. It is being delivered by a consortium that brings together three universities and five national bodies from aged care, palliative care and primary care.

The project seeks to promote changes enabling a reduction of avoidable hospital admissions, with shortened stays, and improved quality of care for people supported in residential and community aged care programs.

What we are doing

End of Life Directions for Aged Care (ELDAC) is a national specialist palliative care and advance care planning advisory service. This service comprises a comprehensive website with resources to equip care providers with skills and information to help older Australians receive high-quality end of life care. National linkage facilitators will continue to work with aged care/palliative care/primary care local partnerships and regional networks including Primary Health Networks to implement a range of evidence-based, sustainable linkage strategies using the ELDAC suite of resources. Learning pathways guiding health and aged care providers to relevant learning resources will be developed and education and training activities will be designed to address identified gaps.

ELDAC uses innovative technology solutions to assist those who work in aged care and palliative care. The tools will provide access to information and resources including online toolkits for those who provide residential aged care, home care and primary care. Other digital and technology initiatives include a digital dashboard that can be embedded by IT companies for use by clients using their IT system, and a careworker app and online self-care tool, both currently in development.
